What details do I need to add for the nominee?

When adding a nominee, please provide the following details:

Full Name of the Nominee

Relationship with the Nominee (e.g., if nominating your father, select 'Father' from the drop-down menu)

Nominee's Date of Birth (DOB)

In case the nominee is a minor (less than 18 years old), you will have to provide additional details about the guardian of the minor nominee: name, DOB and relationship.
